My name is Kristin Clemmer, RN, and I serve as the District Nurse for the School District of Maple.  Please feel free to contact me with questions or concerns.

Please see the medical forms link above to access all medical forms related to asthma, diabetes, allergy action plan, seizures, prescription medication and non-prescription medication.  Medical forms can also be obtained at your child's school.

Parents/guardians are requested to inform the school of any health problem and/or special health care procedures that need to be performed at school.  This will assist the school in developing a plan to best help the student meet his/her needs.

If your child has any of the following:

  • Diabetes
  • Seizures
  • Severe Allergies
  • Heart Conditions
  • Or any other condition that may require an emergency care plan or procedure during school hours please contact me or the student's specific school to address planning needs. 

 


Enrollment and Emergency Information

It is important for us to be able to reach you if your child is ill or injured at school. Please keep your contact information up to date on Skyward, including parent/guardian name with work, home, and cell phone numbers, along with at least two emergency contacts. (Please obtain permission from alternate contact people before placing their names as an emergency contact).  If you do not have access to a computer please request a paper copy at your student's school to fill out necessary contact information.  Also, each year please indicate any medical concerns the school should know about.  Call your child's school with any changes throughout the school year.


Health and Screening Programs

Physical Examinations:  Kindergarten students are strongly encouraged to have Physical, Dental, and Vision examinations prior to school entrance.  

Vision Screenings:  We provide vision screenings for 4k, K,1, 3, 5, 7, and 9 in the fall with the help of our local Lions Clubs.  Screenings are also done at the request of a teacher or parent for all students throughout the school year.

Hearing Screenings:  We provide hearing screening for all Kindergarten students and as requested by teachers or parents.

Dental Program:  Seal-A-Smile Program provided by Superior Smiles from NorthLakes Community Clinic

The program includes oral health education and dental supplies, oral health assessment by a licensed dental professional (Dental Hygienist or Dentist), fluoride varnish applications and dental sealants.

  • Every child is eligible for the program regardless of insurance
  • Superior Smiles will visit the school three times per year
  • A report will be sent home after each visit, listing what services were provided
  •  If decay is suspected a referral will be sent home with information stating what was found
  • Superior Smiles is working with the local community dentists to increase access for care to children with no dental home

Immunization Requirements:

Student Immunization Requirements

State law requires that all children attending a Wisconsin school be immunized against diphtheria, tetanus, pertussis, polio, Hepatitis B, measles, rubella, mumps and varicella (chicken pox). Waivers are available for religious, personal, or medical reasons and can be indicated on the Student Immunization Record form. Medical waivers require the signature of a licensed medical provider.

Minimum immunization requirements for attendance at Wisconsin public school: 

  • 4K: required (4) DTP/DTaP/DT, (3) Polio, (1) Measles, (1) Mumps, (1) Rubella (MMR), (3) Hepatitis B and (1) Varicella. 
  • Kindergarten through 12th grade:  required (4) DTP/DTaP/DT, (4) Polio, (2) Measles, (2) Mumps, (2) Rubella (MMR), (3) Hepatitis B and (2) Varicella. (prior to starting 6th grade, one Tdap vaccine is required).
  • All students must have a varicella vaccine (chicken pox) or have reported having had the disease.  Students entering Five year-old kindergarten must have had (2) varicella immunizations or a waiver on file. 
  • Parents are required to have their children immunized on schedule and keep accurate records.  These requirements can be waived only if a properly signed health, religious or personal conviction exemption is filed with the school.  If an outbreak of any vaccine-preventable disease occurs, students who are not fully immunized may be excluded from school to prevent spreading of the disease. 
  •   When students do not meet the requirements of the law, parents are notified by legal notice and may be excluded from school until they are in compliance with state laws.

Schools are required to keep records and review annually by the Immunization Law (5.252.05 Wis. Stats.)


Medication Policies and Procedures

Please give medications at home whenever possible. However, if medication is needed during the school day, the following is necessary.  All medications will be given in the health office. 

All medication forms can be downloaded under medical forms or are available at your child's school.

Prescription Medications:

  1. Prescription medication form needs to be completed and signed by both the health care provider and the Parent/guardian.
  2. Medication must be in a properly labeled prescription bottle.
  3. Medication must be brought in by parent/guardian, not student.
  4. All medication forms are available at your child's school or can be downloaded under medical     forms.
  5. All medications must be picked up at the end of the school year or will be properly disposed of.

Non-Prescription Medications

  1. Non-Prescription form needs to be  filled out and signed by parent/guardian with instructions for school to give medication.
  2. Medication must be in a non-expired original container. (the medication does not need to be a brand new bottle, just not expired)
  3. Medication must be brought in by a parent or guardian.
  4. All medication forms are available at your child's school or can be downloaded under medical forms.
  5. All medications must be picked up at the end of the school year or will be properly disposed of.

 

Additional things to know about medications:

  • Students may not carry medication at school with the exception of emergency medications such as: prescription inhalers/EpiPen with both doctor and parent permission or a medication release form has been completed.
  • Please have parents/guardians deliver all medication to the health office.  
  • A parent/guardian must pick up medications by the last day of the school year.  They will not be given to the student to bring home.  If medication is not picked up by the last day of the school year we will properly dispose of them.
  • A new medication form is needed each time the medication is changed.
  • A new medication form is needed for each school year.

When should your child stay home because of illness or injury?

  1. Fever:  Greater than 100.4 degrees.  Student must be fever free without the use of medications to lower fever for 24 hours before returning to school.
  2. Vomiting:  Must be free of vomiting for 24 hours to return
  3. Diarrhea:  Must be free of diarrhea for 24 hours to return
  4. Rash:  Rash that is open and draining or a rash with fever.
  5. Contagious Disease:  Strep throat, impetigo, purulent pink eye.  Student may return after 24 hours of treatment with antibiotics and 24 hours fever free.
  6. Nuisance Disease: Head lice/nits (must be treated and checked by the nurse before returning).
  7. Please let us know of any injury that requires your child to have special accommodations at school.
